Service Coordination

ID Service Coordination is the service that helps our consumers with an Intellectual Disability navigate our service system as well as other community agencies. Service Coordination includes assistance in accessing needed services including medical, social, educational and other services. The goal of Service Coordination is to help our consumers obtain a quality of live that is meaningful to them.

Prior to accessing all ID Services, including Service Coordination, consumers will need to enter our services through our ID Admissions process. This process includes gather social history information and what we call a Determination of Intellectual Disability process that will determine eligibility for our services through IQ testing. This process could also include the gather of medical records to determine the age of onset.
