Case Management & Skills Training

Central Plains Center provides an array of services to the people we serve. Routine Case Management services are primarily site-based services that assist an adult, child or youth, or caregiver in gaining and coordinating access to necessary care and services appropriate to the individual’s needs. Intensive Case Management is a service that is available only to children and adolescents. ICM includes activities to assist a child/youth and caregiver/LAR in obtaining and coordinating access to necessary care and services appropriate to the individual’s needs. Wraparound Planning is used to develop the Case Management Plan.

Skills training services are social, educational, vocational, behavioral, and cognitive interventions provided by members of an individual’s therapeutic team that address deficits in the individual’s ability to develop and maintain social relationships, occupational or educational achievement, independent living skills, and housing, that are a result of a severe and persistent mental illness. This service includes treatment planning to facilitate recovery.
